This course offers opportunities to practice the principles and 
theory of community health nursing in a variety of health care 
settings. Included are the identification of health care needs of 
the community as a client, and the nurse’s role as a facilitator, 
teacher, and evaluator in promoting and maintaining health and 
wellness.

Course Learning Outcvomes:

By the end of this course, the student will:

1. Design, deliver and evaluate a community wellness plan using the 
tools of public health and community health nursing based on a 
community health assessment.

2. Collaborate with members of the community and other members of 
the health care team in promoting wellness to an at risk population 
aggregate in the community.

3. Integrate critical thinking in the process of making decisions 
regarding community health care.

4. Initiate effective treatments to promote quality of life and 
relieve pain and suffering in light of patient, family, and 
community values.

5. Examine nursing roles in assuring coordination, integration, and 
continuity of care within and among community resources.

6. Engage patients or designated surrogates in active partnerships 
that promote health, safety and well-being, and self-care management.
